    Physicals, Code of Conduct and Athletic Eligibility Expectations

    Registration Information for All Athletic Programs

    Athletes can register with and receive their red card from the Athletic Secretary Monday-Friday, during their lunch hour.

    To register, every athlete must have the following:

    1.Current Physical Examination- the state of Illinois requires all athletes have a yearly physical examination. Physicals must be completed before a student/athlete can try out or practice with a team. We require physical examinations be completed before the first day of tryouts.

    ***Stratford Middle School Student-Athlete Handbook – This includes general information and expectations.

    An Athletic Handbook is required to be signed by both the athlete and the parents once the team roster has been determined.

    Before an Athlete can tryout, the following items are required to be turned into the Athletic Secretary.

    The participation fee of $20.00 is due prior to the first game/meet!
