Health Services

If your child is entering kindergarten, 6th grade, or is new to CCSD93 (any grade), there are required health forms and, for some students, additional enrollment documents that must be submitted if you haven't already. Please disregard this message if you have already provided all required items to your school.

Required Health Forms 

By Illinois School Code, kindergarten, 6th grade, and all new students (any grade) who do not have a completed Health Examination Form on file by September 15, 2026, will be excluded from school beginning the following day. Most of these grade levels also have additional health form requirements detailed below with separate deadlines that do not result in school exclusion.

Please act now: If you haven't already, schedule any necessary appointments as soon as possible. Most healthcare providers have the required forms on file. The Health Examination Form must be completed by your child's healthcare provider and signed by a parent or guardian on the back.

Birth Certificate & Parent ID (Kindergarten & New Students) 

If your child is entering kindergarten or is new to CCSD93, and you have not already done so, please bring your child's original birth certificate and a parent/guardian photo ID to your school office so copies can be made for school records and additional paperwork can be completed. This requirement must be completed in person and cannot be fulfilled by email.

Complete Requirements by Grade Level

New Students (all grades, including preschool)

• *Health Examination Form: Due by September 15, 2026
• Eye Examination Report: Due by the first day of school
• Child’s Official Birth Certificate (Original or Certified Copy With Seal): Must be brought in person to the school office before the first day of school
• Parent’s Current Driver's License/State I.D.: Must be brought in person to the school office before the first day of school
• Illinois Home Language Survey: Must be completed in person at the school office before the first day of school

Kindergarten

• *Health Examination Form: Due by September 15, 2026
• Eye Examination Report: Due by the first day of school
• Dental Examination Form: Due by May 15, 2027
• Child’s Official Birth Certificate (Original or Certified Copy With Seal): Must be brought in person to the school office before the first day of school
• Parent’s Current Driver's License/State I.D.: Must be brought in person to the school office before the first day of school
• Illinois Home Language Survey: Must be completed in person at the school office before the first day of school

2nd Grade

• Dental Examination Form: Due by May 15, 2027

6th Grade

• *Health Examination Form: Due by September 15, 2026
• Dental Examination Form: Due by May 15, 2027

How to Submit Health Forms

You may choose any of the following options:

• Drop off at CCSD93 District Office (230 Covington Dr., Bloomingdale) through July 31, Mon–Thu, 8:00 a.m.–4:30 p.m.
• Drop off at your child’s school starting August 3 during regular office hours (Elementary: 8:00 a.m.–4:30 p.m.; Middle School: 7:30 a.m.–5:00 p.m.). School locations: www.ccsd93.com/schools.
• Mail to your child’s school
• Email a scan or photo to your school nurse (see Health Services Contact section of this website)
• Fax to your school nurse (see Health Services Contact section of this website)

Please note: Birth certificates and parent/guardian photo IDs for families of new students and kindergarteners must be presented in person at the school office so copies can be made.
