CCSD93 Parents of preschool, kindergarten, 6th grade and all new students (any grade), this is a reminder that you must submit specific health forms for your child for the 2025-26 school year.

*IMPORTANT: Students to be Excluded from School if Health Exam Form Not on File by 9/15

By Illinois School Code, preschool, kindergarten, 6th grade, and all new students (any grade) who do not have a completed Health Examination Form on file by September 15, 2025, will be excluded from school starting the next day. Most of these grade levels also have additional health form requirements with separate deadlines that do not result in school exclusion. See your child’s grade level list below for more information.

Please act now: schedule any necessary appointments as soon as possible. Most healthcare providers have the required forms on file. The Health Examination Form must be completed by your child’s doctor and signed by you (guardian) on the back side. Please disregard this message if you have already provided your health form(s) to the school office.

How to Submit Forms

You may choose any of the following options:

• Drop off at CCSD93 District Office (230 Covington Dr., Bloomingdale) through July 31, Mon–Thu, 8:00 a.m.–4:30 p.m.
• Drop off at your child’s school starting August 4 during regular office hours (Elementary: 8:00 a.m.–4:30 p.m.; Middle School: 7:30 a.m.–5:00 p.m.). School locations: www.ccsd93.com/schools.
• Mail to your child’s school
• Email a scan or photo to your school nurse (check your email for contact)
• Fax to your school nurse (check your email for contact)

Physical Examinations

School Physicals

Physical examinations must be performed and signed by a licensed M.D. (Medical Doctor), D.O. (Doctor of Osteopathy), or nurse practitioner working under a medical doctor. Physical exam forms are available from all school offices and are mailed with registration materials.

Screening for lead poisoning and diabetes are included in the school health physical. Parents of children between the ages of 6 months and 6 years must provide a statement from a physician or health care provider that their child has been risk-assessed for lead poisoning before the child will be admitted into any district-operated kindergarten, early childhood, or child care program. All physical examinations must have diabetes screening completed by the physician. This includes height, weight, and Body Mass Index (BMI). If the BMI is over 85%, a risk assessment is also completed by the physician.

Interscholastic Sports Physicals
Annual physical examinations are required for students wishing to try out for, practice, or participate in interscholastic sports. Additional examinations, including eye examinations, may be requested when deemed necessary by school authorities. All physical examinations must be performed and signed by a licensed M.D. (Medical Doctor) or D.O. (Doctor of Osteopathy) or nurse practitioner working under a medical doctor and dated within one year of the last day of interscholastic sport. Immunizations and physicals are mandated by the state under The Illinois School Code. (105 ILCS 5/27-8.1)

Vision & Hearing Screening

All District 93 students participate in vision and hearing screenings annually.  Vision screening is not, however, a substitute for a complete eye and vision examination by an eye doctor.  Those students whose optometrist or ophthalmologist has completed and signed a report indicating that an examination has been administered within the previous twelve months are not required to undergo the vision screening at school.  Questions regarding the District 93 vision and hearing screening may be directed to the school nurse.

When A Child is Ill

24 Hour Rule

Students who have had a fever of 100º or higher, have vomited, or had diarrhea should be kept home until the fever, vomiting, and/or diarrhea have subsided for a minimum of 24 hours. Students who have been placed on an antibiotic by a physician may return to school 24 hours after the medication is started if they are feeling well. All students with a rash must have a doctor’s note to enter or return to school.

Notification 
Please notify the school anytime between 4:00 p.m. and 8:45 a.m. to report an absence. Notify the school promptly if your child has a communicable disease, including chickenpox, head lice, strep throat, conjunctivitis, ringworm. Students with communicable diseases or undiagnosed skin rashes will be excluded as directed by the Illinois Department of Public Health.

All absences should be called in daily. A physician’s note is required for re-entry to school if the student has been absent for more than five consecutive days. Parents are encouraged to confer with the school nurse regarding chronic health concerns.
