Health Examination Form Required for Your Child No Later Than 9/15

Dear CCSD93 Parent/Guardian,

If you were sent a link to this message, our records show that we have not yet received a completed Health Examination Form and/or immunizations for your child. You have also been sent a personalized email that identifies the child whose form has not been submitted. Illinois School Code requires this form for students who are new to the district or entering preschool, kindergarten or 6th grade. They must be submitted by September 15.

  • If you cannot complete the exam and/or Immunizations by September 15, we will temporarily accept proof of an upcoming appointment.
  • Without the form or proof of appointment by September 15, your child will not be allowed to attend school until it is on file.
  • Families who registered after the school year began have 30 days from their child’s registration date to submit the form(s).

Submitting the form(s):

  • Drop them off at your child’s school office or fax it to your child's school nurse.
  • The doctor must complete the physical exam form, and the top portion, health history, on the front or the back of the physical exam form must be completed and signed by a parent.
  • Forms are available at most doctors’ offices or at www.ccsd93.com/healthforms.

Si se le envió un enlace a este mensaje, nuestros registros muestran que aún no hemos recibido un Formulario de Examen de Salud y/o vacunas completo para su hijo. También se le ha enviado un correo electrónico personalizado que identifica al niño cuyo formulario no ha sido presentado. El Código Escolar de Illinois requiere este formulario para los estudiantes que son nuevos en el distrito o que ingresan a preescolar, kindergarten o 6º grado. Deben presentarse antes del 15 de septiembre.

  • Si no puede completar el examen y/o las vacunas antes del 15 de septiembre, aceptaremos temporalmente la prueba de una cita próxima.
  • Sin el formulario o el comprobante de nombramiento antes del 15 de septiembre, su hijo no podrá asistir a la escuela hasta que esté archivado.
  • Las familias que se inscribieron después del inicio del curso escolar disponen de 30 días a partir de la fecha de inscripción de su hijo para presentar el/los formulario(s).

Presentación del formulario o formularios:

  • Déjelo en la oficina del colegio de su hijo o envíelo por fax a la enfermera del colegio de su hijo.
  • El médico debe rellenar el formulario de examen físico, y la parte superior, historial de salud, en el anverso o el reverso del formulario de examen físico, debe ser rellenada y firmada por uno de los padres.
  • Los formularios están disponibles en la mayoría de las consultas médicas o en www.ccsd93.com/healthforms.
